Waiver All participants must sign a waiver form prior to participating in R.O.C.K. Recreation Night. Waiver forms may be returned upon the participant's first night of participations. In addition, all participants must sign in on the attendance form provided for each day they participate. Download the waiver form and either complete by computer or manually. Please note, your signature must be in writing. Please note, for participants under 18 years, you will be required to complete a ROCK/Beacon registration form. |
